COMPASS™

For hospital and health system leaders

Discharge is the moment a family feels most alone. We make sure they never are.

When a patient is ready to leave the hospital but not ready to go home, your care team carries the timeline and the family carries the fear. COMPASS gives every family a dedicated care advocate and gives your team a documented record of every referral and placement, all in one place.

People lead. Technology keeps the record.

COMPASS replaces the phone-and-fax scramble with one coordinated process, led by a care advocate the family can call by name.

01

Documented

Every referral, every reply from a care community, and every family preference is recorded and time-stamped, from the discharge order to the day the patient arrives.

02

Defensible

When a family, a payer, or a reviewer asks why a placement was made, your team can share the documentation behind it.

03

Equitable

Language, culture, budget, and location are part of every match, so each family is offered options that fit their individual needs.

How it works for your team

Your case managers make one call. We take it from there and keep them informed.

  1. 1
Your team refers the familyYour staff makes one call to our referral line, with no new software to learn.
  2. 2
A care advocate steps inWe confirm the family's needs and timeline, contact them with their consent, and identify care options, such as assisted living, memory care, or board and care homes, that meet the family's needs.
  3. 3
The family chooses, and we stayFamilies are free to choose any community, including those outside of our partner network. We check in after the transition and remain available if their needs change.
Free to families. Partner communities compensate CareMatch when a family chooses to receive care with them.
No payments for referrals. CareMatch never compensates hospitals or hospital staff for referring families.

Why it matters now

Hospitals now report on the Centers for Medicare & Medicaid Services Age-Friendly Hospital Measure, which asks whether older patients' social needs, including caregiver strain and isolation, are addressed before discharge. The fiscal year 2027 Medicare payment year began in October 2026.

75–82%of hospitals face readmission penalties by 2027

How CareMatch helps your hospital

  • Fewer bounce-backs. Placements are matched to the patient's care needs, budget, and location, and the community knows what the patient needs before arrival.
  • Problems caught early. Our advocate checks in at 72 hours and 30 days, so issues are solved before they become a return to the emergency room.
  • Time back for your team. One call replaces hours of phone tag, and every step is documented for your reporting.

Readmission figure per the American Hospital Association. Age-Friendly Hospital Measure per the Centers for Medicare & Medicaid Services fiscal year 2025 inpatient payment final rule.

“My mother cycled through 10 emergency room visits before we found the right care setting. No family should navigate that alone.”
